Our providers participate with most major insurance plans for medical eye exams. You may view a list of insurance plans with which we are considered “in-network” in the forms section of this site.
At this time, we do not participate with any commercial routine vision care plans. We do have an in-house vision care plan for routine vision exams and glasses. This plan is at no-cost to the patient. Please feel free to ask if you may be covered under this plan.
Mountain Eye Associates will file an insurance claim for most services rendered by our providers. Please help us by bringing your insurance card to each visit. Many of our services are covered by your insurance, but there are some services your insurance has excluded from coverage. Because of the various rules from one type of insurance to another, we highly recommend you consult with your insurance regarding eye care coverage prior to service.
Payment is expected on the day of your visit. We accept cash, checks and most major credit cards for copayments, co-insurance, deductibles, and any non-covered services. Copayments are a requirement by your insurance plan and are to be paid at the time of each visit.
When ordering glasses or contact lenses, a deposit of one-half the cost is required at the time the order is placed. The other half is expected when you pick up your order. |
